Corrective actions
Corrective actions are the things you do to prevent the incident happening again. They turn safety reports from documentation into action.
Adding actions
Open an incident report, click Add corrective action:
- Title — what needs doing.
- Owner — who's responsible.
- Due date — when it must be done.
- Priority — Low / Medium / High.
- Completion criteria — how you'll know it's done.
Save.
Tracking progress
The owner sees the action in their My tasks widget. As they work:
- Add progress notes.
- Mark steps Done as they complete them.
- Upload evidence (photos, documents) of completion.
When complete
The owner clicks Mark complete and the action goes to the safety officer for verification:
- Verified — closed.
- Returned — needs more work, with a comment.
SLAs and reminders
Actions with overdue dates auto-escalate:
- Day 0 (due) — reminder to owner.
- Day +3 — reminder to owner + manager.
- Day +7 — escalation to safety officer.
Closing an incident
An incident can be Closed only when:
- Root cause is documented.
- All corrective actions are verified.
- Lessons-learned summary is filled in.
This prevents incidents being closed prematurely.
Reports for management
Monthly safety report shows:
- New reports by type and severity.
- Open vs closed.
- Average time to close.
- Repeat incidents (same area, same root cause).
The repeat-incident report is the one that drives real change.
